Separate Apps for Illinois Endorsement/Temporary

Hello. I am moving to Illinois. Currently have RN in Virginia and will be seeking Illinois license by endorsement. The paperwork is extremely confusing to me. It says a CCA form must be submitted with *each* application. That leads to me to believe I must fill out an entire application for a temporary license, and an entire application for the endorsed license. However, it also says that you can combine the fees into one check. So I'm totally confused. Anyone who has done this before, did you just fill out one application, or did you fill out two?

Also, the verification of employment form. I have been employed for about 6 months. Do I need to fill this out? Or is this just for those that need to prove they have a certain amount of experience?
