Returning to work in PNAP/PHMP program

Hey guys! So I’m about to be finishing up IOP and can now write a letter to PHMP to return to work and I have a few questions! 

  • How detailed does this letter have to be? I got the prompt from my caseworker but I feel like I could sum it all up in a few sentences. It asks for triggers and how to handle them and a relapse prevention plan.
  • If you’ve been through this did it take a long time for them to clear you to go back to work? I want to start applying now since the hiring process takes awhile ya know but I don’t want to have to tell an employer I went through the whole process and can’t start because of this. 
  • Has anyone had a letter to return to work rejected by PHMP? Or is it just a formality?

As far as returning to work, I understand you have to be forward about being in the program and any stipulations but do you have to say exactly why you were in the program? Does PNAP send them anything when you sign the release?
