I know this has probably been asked before, but i need a report sheet. I cant seems to come up with a good one thats small enough to carry around with me haha.
i like sheets with the hours in the shift and then also their info on it.
what are some you use?

We have one we use at work - legal size paper printed both sides so room for 12 pts. This is the postpartum sheet. We have a different one for L&D.
Anyway, it has a space for mother info and next to that, the space for baby info. Mom's info includes name, age, Dr., G/P, vag/CS/Vbac, Bl type, Rubella status, GBS - well you have a space to fill in all this info and more. Wish I could send you a copy. It was done in Word by one of the nurses. The print is small and some of the abbreviations are tricky but it does the job. I started a temp job at another hospital and I take a copy of the report sheet with me since they just use scraps of paper.
