registering in pearsonvue with maiden name

I'm a little confused guys, I wish someone could help me out on what to do.

Situation: I'm currently married without any valid ID which has my married name (greencard still processing) so I'm still using my maiden name (my passport is in my maiden name which is my only valid ID here in the US). My SSN is also in my maiden name because SSA is asking for my greencard so that I could process a change of name. My application for NCLEX-RN is in my maiden name (still single when I applied for it).

Question: I want to register in pearson vue using my maiden name so that it will coincide with my nclex application because I don't know how to process a change of name in my application for nclex in chicago (been calling and emailing them inquiring about it but I didn't get any reply). So when I tried to register in pearson vue, there are questions:

#1 "if you are currently or were married, what's your maiden name?"

#2 "If you ever changed your name, what is your maiden name?"

#3 "If you answered the previous question, when did you last use this name (mm/yyyy)?"

so what am I supposed to do? leave those 3 questions blank? or answer the first and second question with my maiden name but what about the 3rd question, I am currently, still, and will be using my maiden name until my nclex examination.

Well seeing as how you haven't changed your name yet, don't answer 2 and 3. Your name is the same. Many, many people get married and never change their name and many nurses have never been married.

thank you for your reply... yeah, so it means I haven't actually changed my name yet because I'm not yet using my married name.
