I just want to check I am on the right track on my care plan. I am doing "impaired verbal communication" related to severe dementia whereby my patient is often unable to respond or understand questions, and takes a long time to verbalise.
My question is... the care plan itself does not cover daily tasks such as showering/toileting etc, but is more of a general list of interventions related specifically to communication? Other care plans Ive looked at have incorporated the specific problem (eg communication) into all daily tasks which has got me a bit confused.
For example, some of the interventions I have put together:
- Keep questions simple; yes or no questions are often preferable to open-ended questions for dementia patients.
- Give the patient ample time to respond to questions, giving him time to complete his or her thoughts
- If patient is having trouble with word-finding, avoid finishing sentences for him. Be calm and accepting during attempts; do not say you understand if you do not.
Am I along the right track??? These care plans are daunting!! Thanks guys!
