question about rectal tumours

I have a client who has rectal CA.

He has a large tumour that extends about 5 inches around his orifice.

he describes the pain as 9/10 most of the time.

We have tried various creams ..(proctesydl with morphine), (zinc with morphine), xylocaine (it burns aswell) to be applied topically. These creams burn and he gets only minimal relief.

He had a loop ileostomy, that is active, but still has stool that comes through his orifice which he constantly needs to clean.

just wondering if anyone has used any other kind of ointments or has any suggestions.

Thanks.
