I just got the 48 hour walk-throughtemp license. It was really easy! I just brought in the completed application, my valid RN license from another state and my completed fingerprint card. Oh, to get the 48 hour license you have to have a job-offer letter on company letterhead, that says you will be starting your new job in 2 weeks or less! I paid I believe 245.00 all total, (it is more for the 48 hour one.) I brought all that in on Monday around 3pm and went back and picked up my temp license on Wednesday, 48 hours later! I had no problems at all.
No you don't need to get anything notarized, and you can get fingerprints done right down the street for 6.00 if you don't have them already.
I do think this was the most expensive license ever!

Hey all,
I'm gonna try to contact the Board on this but of course they're closed right now. My question is
#1 When you get transcripts for the Board, do they need to be official or unofficial?
#2 For the 48 "walk-thru" deal, do I also hand in the "verification" form then or can I mail that anytime? (I know if you mail the general AZ app, you forfeit your "walk-thru" chance.
#3 Lastly, Does the application in any part need to be notarized?
