Post Op Office Policy in Virginia?

Hi my question is simple. Can anyone who is either an RN or LPN and does office triage in Virginia weigh in on this, actually I would think it should be the same anywhere, but just to be sure, VA. It's a documentation question.

How much charting is standard practice for a phone call. Post op spouse of patient. Would it be standard to take a phone call from a spouse asking for pain med first of all. I think I know that probably happens alot. And if it is a post-op patient 1 1/2 weeks and coming in for an appointment in three days, is it ok to get permission and doc in chart that said per Dr. X, drug (correct drug given, i.e. Vicodin) was called to (and give phone #) and provide dosing info, your sig. Oh also pt. was not on this drug before, had been discharged with different pain med. So I think best practice, would be more info should be there. But am I only in the student world, and is that acceptable or standard policy? Thanks I'm curious.

first i would think that if this patient was not given vicodin on d/c and now she needs it something else may be going on, her pain should be decreasing, i would get her into see the MD before her scheduled appt...
