Hi, I need some help please! I am working on a care plan for a patient admitted to the hospital yesterday with complaints of chest pain. Labs looked good, maintained Sinus rhythm since admit, heart cath was normal. However, he has a history of advanced polycthemia, which is what I am assuming the chest pain was coming from. So my question is, what nursing diagnosis can I use? I know there is an increase for blood clots, but how do I turn that into a nursing diagnosis?

I can see two just by your description, Risk for ineffective tissue perfusion and Acute pain. But your assessment of the patient should be what guides your diagnosis.
