you need to put past employers and reasons leaving, professional references. And your goal sounds kind of rough and cold, put some 'love' and 'care' in to it to show your compassion as a nurse.
Clinical rotations are not such of a big thing to put on your resume unless if the place you are applying at is also the same place you had clinical rotation at and then you would put in how you are familiar how they chart, policy & procedures, and any computer equipment that they use. Maybe make a section of Skills obtained: Also where did you get your home health aide classes or did you have any?
