Personal opinions documented

I’m a retired RN. I am hearing from some friends that are still working in the field, that many nurses are adding their personal opinions of patients in the “note” sections on the Epic system (and other medical software systems). As a result, during shift changes and when patients may come back in for a subsequent visit, they are mistreated based on what was written by the previous nurse. Has anyone noticed that? I have a close friend who did wasn’t very kind to some of her patients and instead would document the patient as “aggressive”, “non-compliant” or “frustrated” to make the oncoming nurses have a distorted perception of the patient before even administering treatment to them. If I were still working, I’d definitely be a whistle blower in that situation. I’m a firm believer that right is right and wrong is wrong!
