Pediatric Healthy Weight Case Management Program (252-847-4151)

The Pediatric Healthy Weight Case Management Program was developed to help Pitt County children and their parents find the care and resources to reach and stay at a healthy weight. The program works to:

  • Raise awareness of the problems of childhood obesity
  • Ensure children throughout the county receive a consistent level of care
  • Support policies that promote physical activity and good eating habits

The Pediatric Healthy Weight Case Management Program uses a multidisciplinary team approach to provide children (up to age 18) and their families with skills and information to maintain a healthy weight. It encourages patients, families and their doctors to work as a team along with a clinical social worker, a dietitian and registered nurses who help manage each child's care.

With help from other specialists, the team develops an individual, written treatment plan to help each child

  • Maintain or decrease body mass index (BMI)
  • Control or improve conditions such as diabetes, high blood pressure, which occur in overweight children
  • Increase exercise and improve diet
