Patient plans, notes ? I can’t be the only one..

Hi so I’ve been a nurse for two years started outpatient recently transitioned to hospital. I work nightshift. Sometimes you don’t always get the best updates or report. I know I should not rely on that either. I find no matter how early I get there I get confused on the plan moving forward. I don’t like the feeling of not knowing or being unsure when relaying to oncoming shift. I feel like discerning the plan from notes is kind of a wild goose chase. Let me know if there’s any tips or tricks. We use epic. Thanks !
