PACU care

I work in a PACU where we do 65-110 surgeries per day. I am revising and updating our PACU documentation guidelines. We currently do VS q15 min for 1 hour, q30 min for 2nd hour and q1 hour thereafter. My question for everyone...do you do dressing checks that often also? What about a back patient. I can't imagine turning a chronic pain, back patient to look at their back every 15 minutes.

What are your suggestions? How is this written into your guidelines?
