Odd arterial line wave form

Published

Pt I recently had showed an odd arterial wave form. Steep upslope, then a dip down and quick bump back up,then downslope with an normal looking diac. notch. Any insights?

Hx,BTW: Failed renal transplant with sepsis, vented,on CRRT. Any significance that I think I heard a split S2 (tho with the disposable isolation stethoscope I had to use it was hard to tell).

My vote is for artifact. I have read of this referred to as a "pre systolic wave" but it is not clear what causes it. I don't think it is related to the S2 as this is a normal sound, the slight difference heard between the closing of the pulmonic and aortic valves. The closing of the aortic valve is the dicrotic notch on the arterial waveform. Any changes or abnormalities on the EKG tracing?

Tele was SR,and this a-line wave had been like that for at least 48 hrs. Still can't quite figure out combination of ventricle/valve/artery variables would show a positive upslope, brief downslope, brief upslope then normal downslope...
