OB careplan help please!!

Please help me. I have successfully done my required three care plans for the quarter clinicals.

However, we now have to do a family care plan with four nursing diagnosis. I am coming up blank.

I just need some suggestions. My baby was very healthy, but they do have some situational issues... They don't have a home and live with the husbands mother. They do have a three year old and an 18 month old.

I have come up with :

ineffective coping r/t changes in family composition AEB sibling rivalry

Parental fatique r/t effects of newborn care on sleep patterns AEB parents verbalizing lack of energy and rest.

interrupted family processes r/t situational transitions AEB newborn addition to the family.

Can I get advice or suggestions please?

is she breastfeeding or bottle, could be something there for infection or inadequate milk, pain from childbirth, overfatigue from dealing with two instead of just one child, transportation or financial issues?

Hi Liebchen. I think you're on the right track - but what about something like "Effective breastfeeding AEB proper latch, no reports of pain by mother..." or "Readiness for Enhanced Knowledge r/t health maintenance AEB...."? You could also toss in "Risk for Caregiver role strain r/t multiple demands behaviors" which could apply to the father or even the grandparents, depending on how involved they are.
