Nutrition Diagnosis and Rationale

I was wondering if anyone could help me or point me in a right directions. I have been given a case study..... A 38 yr old male that has been diagnosis with GERD and history of diverticulosis. No relevant medical or surgical hx. Is currently only taking a PPI (proton pump inhibitor). He is 68 in, 97.7kg and has a BMI of 32.7. Trouble chewing and swallowing. He has a pending colonoscopy and hx of testing which shows narrowing of the esophagus. He is on a high carbohydrate and fat diet and on no supplements. Based on the assessment the main concerns and potential problem for this clients GERD with painful heartburn, dysphagia with pain in jaw and neck. BMI > 30/obesity poor diet choices and diverticulosis.

I now have to give two actual and two potential diagnosis. I have my actual diagnosis as excessive energy intake r/t lack of access to healthy food choices as evidenced by diet hx and BMI 32.7 and my second on as excessive pain r/t difficulty swallowing as evidenced by narrowing of the esophagus. I'm stuck on the two potential diagnosis.......Any help is very appreciated!!!!
