I have an essay i have to write in regards to patient who has CAD and multiple risks factors such as diabetes, hypertension and obesity. He presented to the hospital with chest pain and shortness of breath on exertion and decided to get it check out after a few months. the diagnosis ive come up with are

risk for decreased cardiac tissue perfusion

Activity intolerance

Anxiety

Overweight

Deficient knowledge

Risk for ineffective coping

Im having difficulty with choosing the top 3 and placing them according to Maslow's hierarchy of needs. i think i was able to determine all the main nursing diagnosis for this patient also but i may be missing some

Any help would be great

When thinking of priority diagnoses, think - What will kill my pt the fastest? Also, think of ABCs - Airway, breathing, circulation. So, using that, what do you think your top 3 would be and in what order?

If you included more of your assessment, I could provide more help. It's had to say what other nursing diagnoses you could have and if there are more important ones than what you have without seeing more assessment data.

Depending on the assessment data, I'm thinking there are more important diagnoses you could be using for your top 3.

He didn't really have any other symptoms while in the hospital other then visiting the hospital because within the last few months he was chest pain and SOB on exertion (angina). He did have a sinus bradycardia rhythm of 53 and BP of 144/79. His labs came back with high cholesterol, and LDLs. Hes also been having insomnia. During the shift, he was very anxious about having an angiogram done, and after finding out that he had to have open heart surgery he became tearful and was asking a lot of questions

So far ive decided my top 3 were

risk for decreased cardiac tissue perfusion

Activity intolerance

anxiety

He had to have an angio done - What can result from that? What nursing diagnoses would you relate to that?

He has to have open heart surgery - Why? What is leading to that? What nursing diagnoses could you relate to what is leading to him needing open heart surgery?

Angio can cause a risk for bleeding/hematoma but he had gotten it done before I had my shift there fore I was not considering it in my nursing diagnosis section

Angio results showed 3 blockages therefore from that I got the risk for decrease cardiac perfusion
