I will be starting NS in August. I have been trying to read and become familiar with the material. I have been reviewing my pharmacology book and confused as to how to determine the different phases of the nursing process. For ex: Instruct client not to discontinue medication abruptly is included in which phase of the nursing process? Choices are assessment, planning, implementation, evaluation. What is the best way to learn these different processes? Thanks for your help

Your example would be a teaching situation and teaching falls under the implementation phase.
ADPIE:
Assessment is what you do initially it is like observing the situation, this is when you are taking a look around and determining what the problem is. Remember for testing purposes you always assess before you do anything else.
Diagnose is coming up with your nursing diagnosis these need to be three part for real and two part for risk for and the need to be prioritized (right now you shouldn't worry too much about this you will get into it once you start school).
Planning is just that planning your course of action. This is when you decide what you are going to do, when you are going to do it, and how you are going to do it.
Implementation is carrying out your plan. This is carry out what ever plan of care you came up with, if during assessment you determined the need for teaching this is the phase in which you would teach the pt.
Evaluation is just that evaluating. This is when you take a look back at what you have done and decide if it worked or not. If it worked you are done, if it didn't work then you start back with assessment and go through the process again.
