I am writing a paper due tomorrow on fluid and electrolyte imbalances from a client we selected in clinical. We have to come up with two nursing diagnoses, one for fluid imbalance and one for electrolyte imbalance.
However, we cannot use fluid excess/deficit/ hypo or hyper. Same with electrolytes (no excess/deficit/hypo/hyper).
The topic is CHF, and I was able to come up with one diagnosis (activity intolerance related to decreased cardiac output) for fluid imbalance. Just need some brainstorming ideas for a second diagnosis.
