Nursing diagnoses for my clinical patient

I am having some trouble with coming up with 4 nursing diagnoses for a paper I have to write. I am supposed to:

Provide evidence for and correctly word four nursing diagnoses relevant to the client's health status and wellbeing, as compared to the general population and supported by the research.

Choose & validate two mutual high-priority nursing diagnosis. Describe the rationale for change. Identify the goal, outcome or modification desired.

My patient is 92 years old, she suffers from tinnitus, and arthritis in her knees. Her chart also says that she has dementia, and she is on medication for alzheimers, and anxiety. Other than that I have no idea what to do. She is in pretty good health, she can bathe and feed herself, but she uses a walker most of the time. I would appreciate any help, thank you!
