Would please Help me to formulate Nursing Care For Patient Receiving Anticoagulants?!

After 2 days I have to present this topic :(

So far I have:

- Risk for bleeding R/T prolonged coagulation time associatedwith anticoagulant therapy.

And what are the nursing management that i should follow with this patient?

Here's some things to think about:

The patient is at risk for bleeding.

What are you, as the nurse, going to do in the hospital to reduce the patient's risk of injury/bleeding?

What are you going to instruct the patient to do at home to precent bleeding?

What will you teach the patient about recognizing signs of bleeding?

What lab results must be monitored in a patient taking anticoagulants? What does a high/low result indicate? What nursing interventions would you initiate for a high/low result?

If you do some research about Coumadin teaching and bleeding precautions, you'll find a lot of information.

I also think it would be helpful to start your presentation with a brief overview of the most common anticoagulants (heparin, lovenox, coumadin, etc.), the differences between them, and how they are monitored.
