Nurses and Advance Directives: Brief Survey

I am working on a study entitled "Nurses and Advance Directives" as a Capstone project, and request that you complete the brief survey linked below. The Informed Consent is copied below and also included as the first question on the survey questionnaire.

The survey consists of ONLY 11 yes/no or multiple choice questions, including the consent.

Introduction: You are invited to participate in a research project being conducted by a researcher from Western Governors University. Ronald Zack is conducting research to determine the prevalence of Advance Directives among Registered Nurses and prevalence of nurses assisting patients with Advance Directives.

Description of the Project: The research consists of a survey questionnaire answered by nurses who have been involved in direct patient care for a minimum of 20 hours a week for at least the past year. Other variables such as age, education and length of practice may be considered. Participants will be solicited from Allnurses.com.

Benefits and Risks of this Study: Benefits may be additional information that will contribute to nursing education and patient care. No risks have been identified.

Confidentiality: All responses will be anonymous and all confidentiality will be maintained.

Voluntary Participation and Withdrawal: Participation is voluntary. Participation consists of a brief questionnaire. Once the questionnaire is begun, the participant may withdraw.
