I need some help..advice. I am taking care of a young patient that is WC bound she was on a nother home health at one point, but has switched to us because the other agency was doing the exact same wound care for a year, with the wound getting bigger. so now the would looks better, but its not closing like I would want it to.
she has two stage III on her buttocks...nice and pink...most of the slough is finally gone. We finally go the doctor to change from the sorbsan he used for one year, he went to mesalt...it does look better but not sure what else to do. It took me two weeks to get her an antibiotic for her left wound because it was draining green. and then come to find out we swabbed her wounds like 2 months ago and she does have staph in them. never was treated. so dont know about that. Any other suggestions on a stage III on both sides of the buttocks? we have used sorbsan, now using mesalt...with improvement. but not fast enough for her she says. any suggestions if ya can make them would be great. we also use a barrier cream around the wound because they were really red/raw.

A couple of things....is she on a w/c coushion? where and how big, size wise are these wounds? Is she heavy or boney?? If she does not have a coushion, get her one. If she does, get her a different one...gel works best. Can she relieve any pressure from her butt? If so she should do this every so often. As far as the wound itself, we have used with great deal of success, Allevyn. They come in different sizes. You just put it on without medication. When the drainage from the wound starts to come "through" the pad, change it. Depending on the drainage, depends on how often you change it. I had used it on my uncle and his wound on his shoulder healed within 2 weeks and hospice was amazed with it that they started to use it. It is costly, again depending on the size pad and the drainage, but to me it is worth the money. Good luck.
One thing about my patient is she is medicaide. so its hard to get her supplies. She is WC bound due to MS. She is only 30 years old, so she likes to take her showers every day. so a wound care I guess she likes needs to be done daily. i don't know if I could talk her into 'bed baths' or something like that. to change her to a longer wear wound care product. I will have to check to see if she has a cushion on her WC, I am not sure of that.
Thank you! for your suggestions....how long does that dressing need to stay on? or does it just stay on until you see the drainage? and does medicaid cover them?
I don't know if medicaid covers it you would have to check that out. As far as the length of time that the bandage stays on, it should be changed every 3 days. It really depends on the drainage. I'll tell you what, if you really want to try them, send me a "pm" and I will mail you some. Let me know what size the wounds are because it will depend on the size Allevyn.

Is there depth, Wound Vac, company kci in our area, Dressing change 3 x a week works with neg presssure, hooked to small canister. have healed super large wounds, have used it on many different difficult chronic wounds
