"If it ain't broke than don't try and fix it"
Hello,
I don't want to reinvent the wheel, as I know I can find what I need right here on All Nurses:nurse:. Im in the final stages of getting a Home Care Agency (Non Medical Companion, Personal Care Services) started.
I am looking for forms to assist with:
Client Care Plan
Progress Note
Employee Time Sheet
If anyone has these forms or can tell me where to surf the Net to get them, it would be greatly appreciated.
Thanks,
grouphomes

You can go to providersweb.com for those things.
If you don't mind me asking, how did you get started in the process, how much money did you need to get started etc? Thank you

I have also been researching this sort of business. I am in LA. I am also curious how you got it up and running. Any useful info to share? Thanks.

You can actually a complete non-medical home health startup business package off of ebay for less than 100 bucks with probably 150+ files including how to answer phones, uniform deduction, marketing samples, business plan, and all forms for clients and even those for employees including hepatitis shot forms. It is not everything you will need, but it will have many things that you never thought of.
If you are planning in the future on getting medicare or medicaid reimbursement, the forms will have to be completely redone, and you may run into problems later trying to change out all your forms. Even for non-medical, I would suggest you start with the medical forms required by medicare and medicaid. I actually don't know the best place to get the medicare compliant forms, but you may want to check with local agencies or with a consulting company.
