helloooooooooooo.......
i am so excited that this is my first sebject ...........
first : i'm nursing student
second : i have a problem {of course} that hope u guys help me and this is the specialty for it .......
the problem is i have pt with lumbar spinal stenosis and i need ncp to do sooooooooo
i think about back pain, immobilization or activity intoleranc as dx
and the interventions is soooo wide in my head to degree that i can't find one >>>>stronge i know

The more professional your communication, the more professionally you will be received.
Regardless of the permeation of SMS lingo, "C YA" will still be interpreted as "Cover Your orifice", in nursing forums.
Basically, the question is: how long will you be working with this patient? If you will care for them the next 5-10 days, your interventions should be actual interventions you will perform and assess. If you only have the patient 1-2 days, generally interventions r/safety and/or comfort are relativey short-term(do-able), and then some of it will be "make believe".
If there are no interventions you can identify, that you can perform/assess in 24-48 hours, talk it over with your preceptor, and come up with a couple that you would do, if you were going to be there, and define what you would assess, and how you would determine if they were effective.
As much as possible(through your program) select interventions you will be able to act on and assess for efficacy.
