New Continuous Care Coordinator Needs Help

Hey all, so I just moved in to a new role at a new hospice as a continuous care coordinator and need some help. I'm trying to find a solid example of CC documentation. I've found guidelines and the like but I'm having issues getting it to an easily trainable example format. If anyone who has more time in a similar role has a link to a good example of a few hours of mocked up documentation can you please post the link.
