Need Help With Care Plan Assignment

Hi I am in Nursing school now and I am trying to figure out my care plan for a mock patient. Can anyone help me I have these three so far but don't really know where to go from here.

Risk for decreased cardiac output s/p

Risk for ineffective breathing pattern s/p

Risk for injury s/p hypertension AEB dizziness

My patient was admitted to the telemetry unit this afternoon after spending all day in his doctor's office. He made an urgent visit to his doctor due to complaints of increasing SOB, dizziness and feeling of fluttering in his chest. The doctor did a 12 lead ECG which revealed new onset A-Fib. The pt is a very pleasant and talkative man. He tells you that he is a cancer survivor, having had a prstatectomy, chemotherapy and radiation 6 years ago. He also has a history of hypertensionwhich is controlled with Benicar 20mg po daily. He was recently told by his doctor that he has high cholesterol but it is being managed with diet modification so he hasn't needed any medication. He is a 72yr old African American male. He lives with his wife and adult mentally retarded daughter. He is a retired mechanic and he and his wife provide assistive care for their daughter. I only need three nursing diagnoses but I dont't really understand the process that well. Thank you for all of your help. :confused:

What is the pathophysiology behind A-fib? This answer will help with nursing Dx. Also don’t forget that we provide holistic care to our clients. Do you have a care plan book? Look up some of the nursing NANDA Dx. After reading your post, I came up with:

1. Caregiver role strain (Since the husband is in the hospital, the mother has to take care of the daughter by herself).

2. Social Isolation (His hospitalization may prevent him from attending church and seeing friends and/or family.)

There are tons more…just didn’t want to give the farm away. After reading about the Patho of A-fib, you will easily come up with some others.

How about a knowledge deficit. He has new onset of AFib. He will need teaching about what complications AFib puts him at risk for (and those complications can be added to your risk for injury Dx). Afib will also require new medications which require teaching.
