Need help on a survey on pressure ulcer/injuries prevention

hi fellow nurses, 

I am in an undergrad program that requires me to conduct  a QI project on pressure ulcers. I have posted a survey down below if you take some time to take it I will appreciate it. This will go towards my research. TIA

Do you think hiring more CNAs is cost efficient to lower the risk the risk of pressure ulcers and increase pt. Care?

Do you think conducting a skin assessment before or after will prevent pressure ulcers? 

Yes or no

How does high work load affect your assessment? Do you have time to fully conduct one? Or do you just chart what you see?
