Need Help with Care Plan

I need a care plan with nursing diagnosis for the following case study. I am struggling with this and any suggestions and input would be greatly appreciated. The only thing I can think of is Risk for Infection because the toddler has not had all the series of her immunizations at age 2 and Readiness for Enhanced Knowledge. Thank you, in advance.

The client is a 2-year-old female toddler who has been in good health since birth. The client's mother works part-time; therefore the client stays next door with the neighbor twice a weekly. The client enjoys being at the neighbor's house because her 3 year-old friend lives there.

The client's mother is conscientious about her daughter's health, making sure the client stays home when she is not feeling well, eats a well-balanced diet, gets adequate sleep, and receives her immunizations on time. Thus far, the client has received the hepatitis B, diphtheria-tetorifice-petussis (DTP), Haemophilus influenza b (Hibiclens), mumps-measles-rubella (MMR), and the oral poliovirus (OPV) vaccines. The client's mother recognizes that her daughter will need further immunizations prior to starting kindergarten.

During a recent conversation with her neighbor, the client's mother learned that her neighbor's daughter has not received any of her childhood immunizations. The neighbor fears the foreign substances contained in vaccines will be more harmful to her daughter than the childhood diseases. As support for there belief, the neighbor explained that her daughter had measles last year and recovered without incidence.
