Need 60 day 2009 recert "due" calendar!

I am new to this site, and stumbled upon it looking online for where I can get the 2009 "60 day" cheat sheet, calendar, for when recerts are due. It is the one we use in home care that has the SOC date, then the next column says the 5 day window date, (when recerts are due), and the third column has the cert "end" date. I use it since I am a RN Clinical Supervisor in my home care agency, to advise staff when recerts are due, and it is quicker than counting the days by hand. I went to the CMS site, and couldn't find this cheat sheet. I know someone in home care out there has the 2009 one, or a file I can down load to make my own every year...would you please be so kind to share a link? :icon_hug:

Ok.... I found one at https://www.qtso.com/download/hha/oasiscal2009.pdf, but does anyone have the smaller version, the one with only the first episode dates only, not the 2nd and 3rds on it? Therefore it is only a two pager?

Well...I answered my own question, after looking over the internet! If you go to: http://www.healthcareprovidersolutions.com/news.php?viewStory=44, then scroll down and choose "2009 Episode Calendar" it is all there.

Hope these websites help others with the 2009 change over!
