MSWs doing admissions

We are anticipating having our MSWs doing our start of cares. (Admissions) I would like to hear from others that use this process to make sure we see all the pitfalls before we begin. I appreciate all input, we are trying to find ways to be more responsive to our referral sources with faster response times, Vitas is just killing us here...

We use them if we need somone right away for a referral visit at the hospital. The downfall of it is that alot of these patients require major discharge planning that needs a nurse so we end up having to send a nurse as soon as one is available - otherwise the admission will be a mess. It is easier to use the SW's and Chaplains in the home.
