Hi everyone,

My name is Margaret Sparandera, I am collecting data for my capstone project at on the topic: Assessing the nutritional risks of the elderly at home. Please take a few minutes to complete my 12 question survey.

Introduction

You are invited to participate in a research project being conducted by researchers from Western Governors University. Margaret Sparandera RN, BSN is conducting a research study to determine the barriers that affect the nutritional status of the elderly at home.

Description of the project:

This study will discover the if home health nurses understand their patients risk for malnutrition as well as to determine what tools nurses utilize to assess nutritional health in the home setting.

This study will be conducted via electronic survey presented on the allnurses.com website. Responses will be gathered over a one week period, tabulated and analyzed.

The survey participants will complete the informed consent prior to completing the survey in its entirety.

Benefits and Risks of this study:

The benefits of this study include discovering the barriers the elderly face in attaining nutritional health, learning if nurses are using the assessment tools available to them when assess nutrition in their patients and adding to the body of literature. There will not be any direct benefits to the participants.

There are no expected risks associated with conducting this study, it is voluntary and anonymous.

Confidentiality:

Confidentiality will be maintained as the surveys are submitted electronically and there is no identifying information linking the participants to their responses. Data will be stored in a password protected electronic format that is accessible only to the researcher. Upon completion of my MSN degree all information will be deleted.

Voluntary participation and withdrawal:

Participation is voluntary; participants are asked to complete the survey in its entirety. This will be a forced response survey, all questions require a response. Any time the participant chooses to, they may withdraw from completing the survey by exiting the site prior to submitting the completed survey by exiting the site.
