I dont have extra format and i cant even search the midwifery form here in the internet

ive been to prc to submit my DR cases but they told me that they have a new release form for the nurses wanna take the midwifery exam. Does anyone have this kind of form? please do send it to me. ( [email protected] ) need it badly. thank you so much!!!

how is the application of registered nurses for midwifery?was your application accepted?the BOM is very strict with the cases of registered nurses unlike before.
