medication reconciliation in the community

Hi,

Does anybody have an example of a community outreach program that allows for medication reconciliation and is free to the community? I would like to start a program in my small town. Volunteering one day each month; anybody who has questions about their medication, would like to learn how to fill and use a med box, can come an get help.

I am looking for ideas of other programs, so that I can get the ball rolling.

The South Carolina Hospital Association has a program that I understand is quite successful. It is called the Universal Medication form. For more info go to http://www.scha.org

At my institution we are doing community based med recon and they have a great program that the patient can even logon to. The doctors logon to www.medirecon.net and the patient logs on to www.mymedilist.com.

The forms are good, but this system even lets me print personalized wallet cards when we discharge them.

Give it a try. I think they are not too expensive.

At a free clinic I worked at they gave the patients a form to list all their meds and then a pill bottle to put the form in and then they were instructed to put the pill bottle in the fridge so they could remember where it was. They were told to grab the pill bottle with the info when they were getting any medical consult. They were also given a fridge magnet for EMT or others to know there was a med list in the fridge.
