Medical Records and Insurance Required for Admission Plus ????

I'm starting Collin College (TX) this summer. I'm doing General Biology and Statistics. I plan to get the cna license and start work as soon as my hpb series is complete while working for my adn and bdn etc etc. There are a couple of requirements on the nursing admissions I have questions about.

"Have the Dental Form and Pre-Entrance Medial Form completed by your dentist and physician."

Dear Lord, what do they want to know about my teeth? They're white and that's about the only good thing I can say about them. I'm constantly getting little cavities filled and gum disease off and on most of my life. Also, what is a medial form?

How much health insurance do they require? I currently have none, but it may be possible to purchase a cheap one before I officially apply. I'm assuming this isn't related to the required also needed.

I am searching for this information as well.. does any one know that is in Texas??

I don't live in TX but I also have to include a dental form that just asks if there are any issues and if so what the treatment plans are. The medical forms basically ask the same thing. The doctor fills out a form saying that all systems are normal or not and if they think you are physically and mentally able. The one I had was not very personal. I did have to fill out a form myself that asked if I have and medical conditions and what they were and any medications or allergies I have taken or have. You will also have to get the immunizations and some labs drawn and without the insurance to cover this it can get fairly expensive with the immunizations particularly. My school adds the coverage in with the cost of the program but does not require general health insurance.

I think medial is medical..just missed the C in there. We had to have a physical done as part of our packet, no dental stuff though. I am not sure about how much insurance is required.

I didn't even notice the grammar! I just thought they wanted to know what the medical forms consisted of. :lol2:

There was a post awhile back about the dental issue as well and nobody could really tell why some schools ask for them.
