For your medication record...do IM's need to sign for meds given and / or refused?
Where can I get a MAR with room for several meds on it? I am also looking for a charting system that will work in a county jail.

By "IM"s do you mean inmates? I worked at the Attica State Correctional Facility for 8 weeks for a travel assignment and I don't think so.....you just needed to document a note in their medical record if they refused a med more then three times and any refused insulins and leave the note for the PA or Doc to see. They may decide to just D/C the med or do education if needed, levels if appropriate, etc.
As far as a good system........they had a terrible system and if I was a regular employee I would have loved to see it change.......they used little envelopes and MARS for meds for over 250 inmates. I hated med pass more then any other job there! lol
IM's yes inmates...I think there is a standard that says if inmates refuse treatment they have to fill out a form but that sounds reasonable to document med refused and report if refused 3 times.

In my facility the MAR's are created by using the Excell Program on the computer. We (meaning me) change them & update them every month. Orders change, meds get d/c 'd. The I/M do not touch our MAR's. they are for the nurses to record medications on,as we have three pill lines & Keep On Person Meds.
I don't know if this system would work in a jail, as people are coming and going a lot. But you could try the Excell program to create one. ie: at the top of the page:
First Row: Name & Obcsis next cell, Birthdate, next cell facility name,next cell Month & allergies below.
Next: a block of cells with staff nurses names.titles/a space to initial
Next: Medication
Next days of the week, for the month (ie: 1, 2, 3, 4, 5,
ie( 0800 nurses initials are wrote in the blocks
