Management problems

I work in a rural setting x almost 4 years and I have a terrible manager. It seems everything I suggest to try and improve our team dynamic/relationships and flow she immediately rejects without reason or with dubious reasons. Our relationship is so poor the admin brought us all together (myself, manager, 2 medical directors, manager's direct supervisor) to create a plan to work together and set up the clinic for success. They want us to work as a team (what they call a dyad) and be equal leads on the clinic. Well, she hates this and has done absolutely no work on starting/planning or accomplishing anything. I've done tons of research, reached out to potential training programs, studied the dyad model and research on how to run a good team and she doesn't even respond to my emails, cancels and reschedules our meetings (or makes them remote via phone so accomplishing anything is impossible). I've given her the research I've found and either never heard back, or most recently heard from her "the COO is responsible for finding the program and being the point person". Um, no, we are supposed to be doing it. 
I've reached out to admin and they basically come back to "you'll have to work together on this". I mentioned that I emailed her 4 times without response. Nothing back. 

So, anyway, I think she has an ulterior motive to get me fired (overheard her say to front desk staff, of all people) that I'm a poor fit for the clinic and that I should transfer; that the patients do not like me. Which, not to toot my own horn too much, is patently false. My colleagues in the organization, pharmacist, social work, counseling, RN, have told me I'm excellent, the patients love me (which I know because they tell me so!) and that they wouldn't hesitate to recommend me to family/friends as their PCP. In fact multiple staff are my patients (4). 

My annual reviews are stellar; my med director told me I have the best one he's seen. The coders gush over my documentation (LOL). 

I cannot understand why the organization supports this woman. I am not the only person who has issues with her, and she was even removed as manager at one clinic (yippee gave her more time to focus on me). 

We have follow up with the committee in 2 weeks. I am so frustrated at this point I don't even know how to come off as professional and not oppositional to this woman, which is probably exactly what she wants. I'm considering leaving but have a loan repayment contract for another 1.5 yrs. I can transfer it but it's a pain in the butt to do so. 

Thoughts on how to manage this manager? Handle the meeting? Go about looking for new jobs? I'm considering telling the committee this is unsustainable for me and I'm seriously considering leaving. Should I?

This is an older thread, but- stand your ground. Always be calm. She is out of her depth and you are making her work, which is a threat.
