Lvn california requirements please help!

guys i have a question... iam a graduate of RN in philippines, and planning to take lvn first.. i just want to ask you guys, what are the requirements for lvn?? do i need to submit these forms? RECORD OF NURSING EXPERIENCE , EMPLOYMENT VERIFICATION AND RECORD OF CONVICTION?? thanks
