LPN lost in the world of Team Leader and Supervisor

A little past about myself. I worked as a GPN for 6 months, then shortly after my 19th birthday, I took my boards and passed. It has been a little over 6 years since I passed. I've worked in the same facility, which is a LTC,sub-acute floor within a hospital, for all 6 years, with a couple of per diem jobs @ other facilities, which I quickly resigned. I've worked 2-10 FT for years, many times being "charge" over a 70 bed LTC unit with 12 Sub-acute rehab beds, with a staff or 4 LPNs and minimum of 8 CNAs. @ times, charge of 30 LTC residents with 12 sub-acute patients, AND meds and Txs for 20 LTC patients.

February of 2012, I started working FT 6a-6p, and I could see some issues with the care being provided by the CNAs, none of which had anything to do with patient safety, but more of not providing cares per residents' care plans and profiles. I NOW work 6a-2p, which I absolutely LOVE however, I am having a VERY hard time getting the CNAs to DO there job. Maybe the job gets done, but then their documentation is lacking. Or, they will get people up for the day, whenever they get to them. That puts their medications behind, that puts me behind, which comes full circle and puts the CNAs behind because I may need their help for something with said resident, or another. I know if I feel things aren't getting done properly,safely- it's my job as a "Team Leader" to handle it, and follow with disciplinary action if necessary, but if I do- they refuse to sign and they have a "grudge" against me for at least 24 hours. I have a Charge RN above me, and a Nurse Manager, but it seems like the MORE staff, the WORSE people behave. If I talk to the CNAs about an issue, it always turns into an argument. To be honest, I think it's a bunch of Bull.

I'm at my wit's end at how to deal with all of it- meds/treatments for my residents, Dr Orders the RN just throws on my med cart, people going off the unit to go downstairs for x-rays/CTs/ultrasound, reidents' families, trachs, Gtubes, but the hardest and worst to deal with - staff who don't do their job.

I just wanna SCREAM @ the top of my lungs, CERTIFIED NURSE ASSISTANT- I know you are HERE for the resident's not me, but your job title does say, Nurse Assistant right? Shouldn't that mean-ASSIST NURSE?

Any one have any ideas on how I can make my team better?? When i worked afternoons, the CNAs weren't the best, I'd write them up too if need be- but they would take responsibility- owe up- and sign it. Not go up and above me to gripe about it.

I don't want to be the "B****" nurse, I just want us all to work, and do our jobs as it should be done. Is it too much to ask for?
