List of Jobs on Resume

I've been a nurse coming up on four years now. Before that, I was a CNA. I've maintained my close connections with the management at my old CNA job---the only reason why I didn't end up working as an RN there was because I moved away to take care of family while starting my nursing career. Should I keep my CNA position on my current resume under my work history or does it get the boot? In order to include it, I need to stretch my top/bottom margins a little, because my skills section takes up so much room.

Also, right after graduating, my first job was at a SNF that only lasted for 4 months due to a 40-80:1 resident:nurse (ie. me, without more than 3 days of orientation as a new grad) ratio. What is the lesser of two evils: including that 4 month job on my resume which makes me look like a job-hopper, or leaving it off (which makes it look like it took me 6 months to land my first job after graduating and moving)?
