I have 4 case scenarios and I have to create a plan of care for what I will do on my shift with the time frames.
1.Pt who had TURP
2.R hip replacement
3.chloecystectomy
4.C-section
It's so hard to make a plan when you haven't been in the hospital

I have this for the R-hip replacement
Plan of Care
0700-0900review patients chart, check ID, perform: Neurovitals, Glascow's Coma Scale and Head to Toe Assessment, check V/S and assess the 5 P's, administer medications
0900-0945assist with bed bath
0945-100015 minute break
1000-1200check V/S, provide passive ROM on affected limb and have patient perform active ROM on unaffected limbs, toilet client with slipper bed pan and have client perform kegals exercise
1200-1230lunch break for 30 minutes
1230-1300check V/S and Neurovitals, assess the 5 P's, administer medications
1300-1400 provide passive ROM on affected limb and have patient perform active ROM on unaffected limbs
1400-1445toilet client with slipper bed pan and have client perform kegals exercise
1445-1500document in fluid balance, flow sheet and nursing notes
This what I have for C-section (I forgot to mention she is on discharge day)

1000-1200check V/S, have patient perform active ROM, toilet patient, have patient demonstrate a side-laying hold for breastfeeding
1230-1300check V/S and Neurovitals, assess the 5 P's, administer medications, have patient demonstrate a cradle hold for breastfeeding
1300-1400 have patient perform active ROM
1400-1445review discharge plan with patient, ensure patient has a proper car seat and demonstrate how to set it up in the car and safely put the baby in seat
1445-1500 document in fluid balance, flow sheet and nursing notes
