How VA PACT Nurse functions?

Hello ~ could someone explain the RNs role as a VA PACT nurse? Are you providing direct care (taking vitals, giving meds, changing dressings) or are you more of a case navigator (more administrative) - or a combination of both? Despite researching the position, I'm still not really sure what the actual job entails.
