How to renew an expired Visa Screen?

Hi,

I am a nurse from Canada and after passing the ENCLEX,

I was given my nurse's license from Florida in 2000.

I didn't renew my license a year and a half ago as I retired.

Since then,I changed my mind and I would love to work again.

For the license,I got the information.It is my visa screen the problem.

And of course,I didn't renew my Visa Screen either, so I would

appreciate very much if you could give me the information

needed to renew my Visa Screen.

I went to the CGFNS site and all I could see is that I have to fill out

a form which is asking me for details I don't have such as details about

NCLEX(results I never saw) and my TOFFEL test etc.

I also made a search on Google and All nurses site.

Somehow I was not able to see what I need

Do you have an easier way to do it?

Could I just asked them to look me up with my social security number?

Or send them a copy of the Visa screen they sent me before?

And of course pay the fee attach to it?

I don't see myself going through the same process I went through years ago.
