How do I properly fill up IL's Continental Testing Services' online application?

Hi everyone! I'm currently filling up Continental Testing Services' online application but I'm unsure about the "Record of Licensure Information" and "Record of Examination" sections.

I already passed the Philippine Nursing Licensure Exam and currently hold a Philippine Nursing license, so do I have to include this information to their respective sections of the application?

I've emailed and called CTS several times last Thursday and Friday but their lines were too busy and they still haven't applied to my emails.
