I don't live in a huge town, but how do you find these jobs? Are they only available in big cities like Chicago?

You can check with your local hospital regarding UR positions. Otherwise, it sounds like you may have to travel a distance to work with an insurance company for an insurance job or UR. Some employers are going remote with UR and insurance jobs. If there is a United Healthcare within your vicinity, that's one employer that comes to mind you can try.
