How to apply for NCLEX pass letter with photo

Hi,

I would like to ask about the process of requesting for an NCLEX pass letter with photo from the California BON.

1. To whom should I address the $10 draft to?

2. What should the letter contain?

3. To what address should I send it to?

If anyone can send me a sample request letter it would be appreciated.

Usually you will make it out to: CABRN or maybe CA Board Of Registered Nursing. It's pretty self explanatory, just ask them for a copy of your NCLEX pass letter with a photo. Make sure you include a copy of your ID/DL so they will know it's you. They may not request it, but it's good to be on the safe side. Also include any numberical/alphabetical identification that they assigned you for your exam. Also make sure you have them send it to the address where you're located. You may also want to send a self addressed stamped envelope. If unsure of the postage then go tot he post office and ask for an extimate. Send it to the CABRN website in Sacramento. Good Luck
