Hospital Discharge planning Orientation

I am working on revamping our orientation process for discharge planning Case Managers. I have a pretty good list of competencies, but I am really looking for a process. For example, for the first couple of days the new hire observes, for a few more days he or she manages 2 patients. The next week, 5 patient and is independent in daily rounds… etc. Please let me know if you might have anything to share.

Are you speaking of hospital Med/Nurse Management? And is the person getting orientation a seasoned case manager in DC planning with similar processes?

I have only done HH case manager work, but based on what I have read from CMs posting on a more active CM site:

I hear they are all different Hosp based CM, WC CM ,pediatric, UR /CM with insurance companies. I have read some do not feel really comfortable until a year and the training is sometimes 6-8 weeks or longer.

I would want to read through the process first by myself, or with a real teaching orientation: powerpoint or mix with a video. I personally would not want to have 2 patients the first week unless someone is right there to help and be truly available--not trying to cover their work and help the new trainee. That usually makes the trainee feel vulnerable bc the learning curve can be 6-9 months esp if new to CM or switching from another area of CM..
