This is my first post but I’ve been reading this site for many years now. I have a new position within our small non-profit hospice and it will require justifying staffing for budget purposes among other things. Not only do I find it necessary to support nursing staff for patient safety, I think it makes for a happier workplace when nurses feel like they can spend the appropriate amount of time with their patients based on needs. When I worked in a hospital as a charge nurse I had a chart for acuity and assigned patients accordingly. Since being in hospice I’ve not seen them being used but think it would be great to implement.
Does anyone have an acuity scale/chart their team uses that they find helpful? I want it to be succinct and easy to understand so that we have everyone filling it out the same way and not leaving it up to interpretation. Say I have 2 nurses and a nurse tech working in the in patient unit and there are 6 patients. One patient might be GIP and have TPN, uncontrolled symptoms like pain/dyspnea, total care and one might be actively dying requiring q1hr IV pushes. One might be walking and talking and independent. It might be good for staffing to say the patient 1 is acuity of 5/5 and patient 3 is 1/5 or whatever. Sorry for rambling but I don’t know how to logically create a chart that works and can justify needing 2 nurses and a nurse tech on a given day. I also need one for the home care team to justify their 8 hour shift if they’re only seeing 4 patients that day.
