would like to know if anyone has sample forms/questions that patients/relatives complete regarding the hospice care that their loved ones received during the palliative/terminal stage of their illness.

Hi Tommy! Nice to meet you. I work for a hospice in New Jersey. Our hospice uses and outside agency who mails out surveys and compiles the data. I personally don't like the survey, I think some of the questions are awkward and don't give us any valuable information. But I will try to get one of the surveys and give you the questions.

Did you know that there is a hospice forum here? Your question might get more answers on that forum.

Iwork in aged care and palliative care here in Australia and refer a lot to the site http://www.palliativecare.org.au as they have a lot of information that could be a help.

There is another comprehensive site that you could refer to that may be useful:

http://www.pallcare.org.au There are sections that give plenty of ideas but no actual print-out questionaires. You could try the publications index on that site.

I know these are very Australian but I don't know of any other at this time.

I am sur there must be something in the UK, if I find anything I will contact you again.
